What is collected when you book

The booking page asks for your name, and optionally your email, phone number, and a short note about what you would like worked on. That is all it sends. Your request goes to the scheduling service Caladrius Bodywork uses (Passel), which holds it until Elizabeth confirms the time in her app. Passel receives nothing from your health form, ever.

Your health form

Before your first session you complete a health-history and consent form, either online at the health form page or on paper at the studio. It covers the things that make bodywork safe and useful: injuries, surgeries, medications, allergies, pregnancy, and conditions that affect how a session should run. Only your name and the consent are required; everything else is up to you.

A form submitted online goes into a private records system, separate from the scheduling system, hosted on Google Cloud under a Business Associate Agreement and encrypted in transit and at rest. It is readable only by Elizabeth, signed in with her own account. Your browser cannot read it back after sending it, so a shared computer never exposes it. The form is never emailed and never passes through any third party.

Session notes

Elizabeth keeps a short clinical note after each session — what you reported, what she found and did, how you responded, and the plan. These are professional records, kept in the same private system, and are never shared without your written permission except where the law requires it. Every time a record is opened, that access is logged.

Your health information is treated as confidential under HIPAA's standards, professional ethics, and Virginia's health-records privacy law, whether or not a given rule technically applies to a small practice.

What is never done

• Your information is never sold, rented, or traded.
• Your information is never shared for advertising or marketing by anyone else.
• Health details are never discussed publicly — including in replies to online reviews, where a client is never confirmed as a client.
• Records are shared with another provider only with your written permission, or where the law requires it.

Tracking and cookies

This site runs no advertising trackers, no third-party analytics, and no social pixels. Fonts are served from this site rather than a third party, so simply reading these pages sends your data to no one. The health form and payment pages sign your browser in anonymously to the records system so it can accept what you send; that session is not tied to your identity and is not used to track you. The only other browser storage is a short-lived note that carries your name and email from the booking page to the health form so you do not type them twice. It is cleared once used.

Email and messages

If you email, call, or text, that message is kept only as long as it is useful for your care. Please do not send detailed health information by email or text — bring it to your intake form instead, where it is stored properly.

Keeping and deleting records

Client records are retained for as long as professional standards and state law require, and securely destroyed after that. You can ask to see your records, correct them, or have them deleted where the law allows — just ask.

Payments

If you pay online, checkout is handled by Square on Square's own pages. Square receives your name, email, and card details — never anything from your health form — and this site never sees or stores your card number. Paying by card at the studio goes through Square too, so both appear on one receipt history. A record that you paid, with the amount and date, is kept alongside your booking. Paying at the session, by cash or card, works exactly as before. The cancellation terms (24 hours' notice) are on the FAQ page.
